Hearts at Home provides comprehensive care
for loved ones who require professional home care as a
result of a variety of situations including Post-hospital Discharge,
Traumatic Brain Injury (TBI), Stroke, Paralysis, and other chronic medical
issues.
To maximize quality of life and ensure continued recovery at home, our Care Managers will consult with the client, family members, and medical team to develop a personalized care plan. When appropriate, we make every effort for the care team to undergo training with the rehab team to ensure a successful implementation of the prescribed plan of care.
